Alternative output.xsl for MySQL

Nice job, but I am afraid I cannot replace the default one with yours, because:

1) you removed the sql COMMENT clause,
2) the default dummy data might not be suitable for every user.

Fair enough - modified version attached. It would be nice in the future to have 
the
capability to add globals, table types and dummy data.

Nice, commented declarations make more sense. However, I am still missing the 
COMMENT
clause, present in the original XSL template. Is removing it an intention or 
just an
oversight?

The comment clause for tables is there - it only appears if a table has a 
comment
assigned however.
