Full integration of Issue Template #282

This PR promises a full use of the Issue Template for any newly created issues. Benefits of having an Issue Template result in faster bug triage and feature request process.

If you have to make any edits to the Issue Template, please do it in this PR and then click Merge. This will generally make my life simpler.
