this should be implemented as --tn 0/track_count when running LAME

This would be nice, but you won't get any track metadata that way. Instead, I'd recommend you manually adjust the cue sheet to have a TRACK 00 AUDIO directive to cover the HTOA pregap, that way you can also apply any necessary metadata.
