Design Bug -- Ability to add new contact in shipping module/schedule shipment

What steps will reproduce the problem?

What is the expected output? What do you see instead?

Please use labels and text to provide additional information.
When scheduling a shipment user should be able to add a new contact.
Suggest adding a select option, "add new contact" to drop down menu in
"ship to company", which opens up blank address & telephone fields.
