Record vaccinations

As an authorized user When an animal receives a vaccination Then display the following information on the data input screen:

• PAR ID Number
• Animal name
• Animal's DOB
• Animal's photo

and record the following information:

• Date of vaccination
• Vaccination type
• Date when the next vaccination of this type is due
• Who gave the animal the vaccination
• Whether the vaccination was pulled from the rescue's inventory

@ktody, we need either a list of all of the vaccination types and/or a story that tells me how to create new ones. Also,

• do we have dosages? - No, vaccinations do not have dosages
  • Based on what?
  • How is it determined?
• Are there generics? - Yes, I will add a generic description for each vaccination type as well as the brand we use at the rescue
  • Do we use the brand or generic name?
• do we care where the vaccination is given? - No, we do not track this information *subcutaneous
  • intramuscular
  • interveinous?

This is going to end up being a number of stories, I'm guessing.
