🐞[Bug]: Improvement in the Make an appointment Form

Describe the bug

The form of make an appointment is not looking nice so can I fix this issue .

Expected behavior

If the width and position of the make an appointment form will be changed so that looks good and improve the UI of the website.

So please Assign me this issue with the GSSOC Label and level also , I would like to work on it
