Removal of unnecessary boxes

Feature Description

There is an additional box beside of date of birth, which is unnecessary and not required it can be taken out and only date of birth can be kept

Use Case

It will provide better UI

Benefits

It will provide better UI and removes unnecessary confusions for the user
