Major Overhaul of Bug Report Template #302

As you probably noticed, whenever I submit a bug report, I use my own custom template. Well, I figured it was time to finally implement some of those changes into the official bug report template! (If you want.)

With this overhaul of the Bug Report Template, instructions are given for some of the more troublesome fields ("Login Type", for example), the form looks much nicer and tidier, and of course, we finally thank the users for their contributions!

Feel free to further modify this renewed form if you feel it's missing something or if something still seems confusing. Hopefully, though, it's nice, easy to use, and crystal clear!

(Note: Please ignore the "bug_report.yml" file nonsense. I was experimenting with something. That has been moved to a different branch of the fork.)

Eh. This is a little outdated now. Plus, it seems like it's never going to be reviewed anyway, so…
