Update Mokka to Qt5 #64

What should be the improvement?
Qt 5.0 was released in December 2012 and Mokka should be adapted in consequence 
as Qt 4.8 won't be maintained anymore. However, this means that some part of 
the code will change due to the modification in the API of Qt. Phonon is no 
more included in Qt and was replaced by QtMultimedia. VTK 5.10.1 doesn't 
support Qt5 but it seems possible to pick the required files for QVTKWidget and 
adapt them in consequence.
