fix/remove-binary-packs-files

Simple housekeeping PR:

• Update gitignore for vscode and packs folders
• Remove generated packs files from the index
• Leave the source json's in the index (so that we can regenerate the binaries locally)

This will mean that we can generate packs files for publishing, but not have to deal with them clogging up our working tree.
