Define upgrade process from v2 to v3 #85

Support versioning of content hashes and canonical hashes
Require upgrade to the latest 2.x before upgrading to 3.0
We are converging on using the import-export feature, with the possibility running an offline upgrade tool on the exported data (for e.g. hash upgrade), accepting some amount of downtime.

Another possibility is to accept a V2 export zip and re-calculate all the hashes using the existing algorithms in V3 at startup time. This would remove the need fora CLI tool.
