Create class 'disease surveillance' #109

Per #96, and for the indexing tree, we'll need a class with rdfs:label 'disease surveillance'.

As such, using the wording for the definition of 'disease surveillance data item', I propose we define 'disease surveillance' as: "A planned process that is the realization of a disease surveillance objective specification."
