Add US to list of countries for Agency & Supervisor #267

The United States is not listed in our set of countries. This is to prevent users from creating an international internship, and then choosing the US.

However, when an internship is international, there's no way to give an agency or supervisor address in the US, because all three address block switch to "international mode" and US isn't in the list of countries.

Add 'United States of America' to the list, but only for the Agency and Agency Supervisor fields.

Be sure US is not included in the list of countries when it is used elsewhere (for internship location).
