LASzip 2.2.0 incompatibility #29

@r-barnes reports that PDAL does not work with LASzip version 2.2.0, but does work with version 2.0.2; and requests that we update the install doc correspondingly.

Our current installations at AGS are using

icooke@rio:~$ /usr/local/bin/laszip-config --version
2.2.0

@ircwaves does that confirm that lidar2dems is not functional at AGS?

I guess my question would be: are you, @F-Sullivan, or anyone at AGS using l2d on rio?

I am currently running a test install in a fresh ubuntu 14.04 container on my laptop. I will post back the results.
