As Kev mentions in his 1994 comments in missing.h, the MISSING_EXTERN mechanism was a temporary patch until Unix systems fully adopted ANSI C (C89). I think 25 years is enough to wait...

Agreed. Waiting past c18 ought to guarantee adoption of c89.
