Error 404 #35

I have problem with votes when i click on it its gives me this

and cant log out the same error appears and similar with some of other pages ???

see URL!!!...should be http://yourdomain/account/vote.php <--- correct should not be http://yourdomain/vote.php <--wrong i not found this problem in CMS
