As far as I'm aware, most professionally developed games have documents! I was allowed the honor of looking at the game design doc for Intrepid Studios.
What is needed to implement the task?
A good understanding of the game's design, and google docs.
What is the biggest risk it will not get done?
If I can't understand what we're trying to make, or cannot compose a coherent document that everyone will be able to understand, the document isn't useful as a design tool.
How do I know it is ready?
When I can hand it to one of my teammates, or even a stranger, and they can understand what the goal of our development is.
