something like https://www.typeform.com but fully decentralized. data collected directly put into user designated places (did storage, google drive, sheet, etc. )

lots of open source one.

TypeForm is very expensive. $20/month for a simple form, free version almost useless.

最新form builder 可以考虑和xmark的结合，用户很容易设计form 并收集数据
