Simplify all the custom exceptions and put them under an `AO3Error` umbrella exception. #94

I saw a bit of repetitive code in the custom exceptions and figured I could provide a version of them that's a little less so. Apologies if the commit looks a bit odd; I didn't properly do it in a way that's the best for viewing.
