Add "project" - "story" - "epic" milestones to next issues #21

  1. We now hose any issue under a milestone or create one. These are our epics to keep trak of.
  2. We now have a (to be edited at will) issue template for our core team team with
    • a short task descriptiopn.
    • defaults to the core-setup project
    • adds a needs refinement tag - to review in sprint planning if needed, or to be removed.
