with black screen

MPC-HC Splitter + LAV Video with DXVA2 (copy-back) = file plays correctly,
falls to software rendering
MPC-HC Splitter + LAV Video with DXVA2 (native) = file plays correctly, falls

MPC-HC Splitter + LAV Video with NVIDIA CUVID = file plays, only audio working,

I think LAV Video with NVIDIA CUVID didn't fails to software rendering properly
in some cases

Using MPC-HC 1.6.1.4117 with updated LAV Filters 0.48 (Splitter, Audio & Video)

This is a driver issue.
Either disable MPEG4 ASP support for CUVID, or install a different driver. The
285 driver seems to work.

PS:
There is a reason MPEG4 ASP for CUVID is off by default and needs to explicitly
enabled. ;)

I can confirm this. I use latest 320.00 beta drivers and it didn't occur.

This bug can be marked as WORKSFORME instead of WONTFIX :)
