remove "Professional growth plan" from Team Facilitator

We no longer do Professional Growth plans for TFs (well we never did them in the 1st place). Go and and remove

• Last Facilitator Professional Growth plan from the page layout and
• Professional Growth Plan as a type for File upload for TF's.
