move CGC Form to "Team Dog" from "Team Client"

If this is a pain, let's not do it, but realizing CGC form is about the dog, not person. so it would be on Team Dog vs Team Client. If this is a pain, don't bother. if it is fairly simple, it's cleaner to move.
