Database schema #63

It would be really useful if an up-to-date database schema was included with the project, to help sysadmins who are upgrading existing implementations. I've run into a number of 'bugs' that are really just database changes compared to older versions.

Work around is to apply database migrations from ALA CAS 5 project here https://github.com/AtlasOfLivingAustralia/ala-cas-5
