Add an AppVeyor config #26

Attempts to download and install Composer, then install phpmd using that.

AppVeyor failing is an Atom bug, I'm not sure what is going on with Travis, something about crashing due to there being no spec folder.... I'll be updating the build script for Travis in a separate PR.
