ValueSet – Service Section – support exchange and categorisation of Diagnostic Reports #112

We need to develop a set of terminology that supports the exchange and categorisation of diagnostic reports using the concept of service section.

A service section is a part of an organisation that provides services, e.g. a pathology section provides pathology services. The clinical community organises some clinical work and work groups (e.g. pathology) by the type of service undertaken, thus service section is one of the ways that the work is categorised.

The resulting value sets will be bound in DiagnosticReport profiles to support exchange of diagnostic reports issued by diagnostic service providers for the electronic exchange of diagnostic reports (e.g. pathology, imaging, specialist and other) between healthcare providers, and between healthcare providers and the My Health Record system infrastructure in Australia.

There will be 4 value sets, an encompassing one for all diagnostics and limited value sets for each of imaging, pathology and specialist diagnostics.

Terminology and modelling choices will need to consider a hierarchy of domain concepts. For example: Clinical → Diagnostic → Pathology → Anatomical Pathology

The values should, if possible, be drawn from v2 DIAGNOSTIC SERVICE SECTION ID. However, it is not well aligned for specialist diagnostics. There are some non-pathology and non-diagnostic imaging options including "Nursing service measures", "Occupational therapy", "Other", "Pulmonary function", "Pharmacy", "Physician" and "Physical therapy". Work will need to be done to determine more appropriate values.

This will help diagnostic reports other than pathology and imaging ones (though imaging does need some work) to be exchanged as effectively as pathology and imaging ones are currently.

There is no good workaround for not having these value sets.

This set of codes is fundamental to exchanges of pathology information using HL7 v2 and is ubiquitous in Australia. It is weak outside pathology.

METeOR has codes for specialty, but for organisation unit the closest is Clinical service context code N[N] with 32 values. It has a single code that is relevant: 8520 - Pathology and Diagnostic Imaging Services

The criteria for suitability looks to place significant emphasis on whether a service section 'is a' domain and at the same level of granularity by which the domains have been decomposed. Suggest there is some conflation here... or further explanation required on why this is a useful view of service sections. I am very cautious about this:

To those points above I am generally of the expectation that a code such as "OTH" or of equivalent nature is an important desired feature though this largely depends on the principles underpinning the value set designs and expected behaviours / extensibility.

NOTE: There are four value sets requested to be created, suitability needs to be with reference to the four sets. That suitability against the four requested sets is difficult to determine in the current layout.

The following summary was made following a discussion with Brett Esler (@oridashi) when discussing AU Base GitHub #415:

FYI - NCTS value set created 2020: ValueSet : Pathology Diagnostic Service Category.

Feedback from imaging people is that table 0074 is not a good fit for imaging and for the specialist and other diagnostics that is not well accommodated by HL7 Table 0074 - refer to ValueSet - Diagnostic Imaging category terminology for DiagnosticReport #55.
Hence work is going on to develop value sets for imaging and specialists and other diagnostics - refer to ValueSet – Service Section – support exchange and categorisation of Diagnostic Reports #112.

@oridashi is happy to have content referred to NCTS content which allows extensions and change which is tracked.

@oridashi suggested the best options would be to have value sets for: 1) pathology - done 2) imaging #55.
3) specialist and other diagnostics #112 4) a value set combining all of 1-3 above.

@oridashi suggested to create a combined set (4) with just the content available i.e. pathology and add content (imaging and specialist) when it becomes available. This would enable a value set to be used and become more useful as content is added.
Would need to confirm if the uri number would change with added content or if the uri number changes only with a breaking change i.e. content is removed from value set.
