Change description
Design and develop a set of materials that define a Clinical Information Object for clinical frailty for the electronic exchange of aged care information between healthcare providers, and between healthcare providers and the My Health Record system infrastructure in Australia.
What are the use cases we want to support?
recording a narrative statement about a person's clinical frailty
recording an observation of a person's clinical frailty as assessed using a frailty assessment scale
one such scale is the Canadian Study of Health & Aging, Clinical Frailty Scale, as endorsed by Clinical Excellence Queensland[^1]
At this early point based on my own preliminary research on the matter, no particular assessment tool/scale seems to be universally adopted across Australia.
Clinical frailty information is not currently implemented in the MHR system or explicitly supported in existing Agency specifications, the closest material in nature is that of Problem/Diagnosis (i.e. Condition) but no grading, staging, or assessment elements are supported.
