US-Core requirements need to become jurisdictional #80

Given that the SANER IG is now international, the PSS got approved that way, there should be no US-Core profiles.

Which would also clear up that Location as an Identifier can't possibly be tested to see if it is a US-Core profiled Location resource.

CDC and FEMA Measures need to have a US Jurisdiction, but can be specified in Measure.jurisdiction. US Core constraints might be assumed by a MeasureReport that references a Measure with US Jurisdiction, mechanism needs better explaination. There's a whole in FHIR where Definition/Instance constraints are not well identified, nor computable.
