Change breadcrumb and URL to match changed topic name #509

Apologies if this has already been reported.

Problem:

If an admin/author changes the name of a topic (within the Admin area of the Platform), the corresponding breadcrumb and URL remain unchanged.

(I'm getting deja vu with this – is it just a caching issue?)

Suggestion:

When an admin/author changes the name of a topic, the corresponding breadcrumb name and URL should update at the same time.

Note: I can see pros and cons for admins/authors being able to change a page URL so easily, so some extra consideration may need to be given to that.
