Revise README.md to rework the landing page for the VTT plugin.

This becomes more relevant now that we understand how the Forge hosting site will auto-generate its Bazaar listing based upon the contents of the README page.

Task in general is:

[ ] Create a "product image" to be used on the page
[ ] Replace the initial opening Header with the product image
[ ] Rework the opening paragraphs so that the Bazaar listing gives a better description up front

Definition of Done:

The work identified above has been completed.
The Forge listing for ACKS looks "better" - this may take a few days to refine the look, since they pull updates once a day.
