This can be done with WP.com "Follow Button" creation: http://developer.wordpress.com/docs/follow-button-creation/ — but would be even better as a Jetpack widget.
I was thinking about the other follow button, appearing at the bottom of WordPress.com sites when logged out, as displayed on the plugin page I linked to above.
Just noticed that the follow button generator doesn't work if the site has JSON API disabled.

Reopening as the Follow button added in #5615 isn't the one that's mentioned here.
