Frequency settings panel #3

From CS-16 for WE-19:

After #2, write a general settings panel that allows the user to configure defaults for the frequency of placements appearing in post content:

  • [x] shortcode appears after first N paragraphs
  • [x] shortcode appears every N paragraphs for X times.

State of the 3-settings-panel branch right now: Settings page is registered, but forms are not outputting.
