Update environment variables and documentation for Azure OpenAI and JIRA configuration

Rename AZURE_OPENAI_DEPLOYMENT to AZURE_OPENAI_LLM_DEPLOYMENT_NAME and add AZURE_OPENAI_EMBEDDING_DEPLOYMENT_NAME in .env.example
Remove single quotes from JIRA_API_TOKEN in .env.example
Add step to install setuptools in README.md
Update environment variable references in utils.py to match new names
