Comment for Measurement #17

A user requested the ability to enter a comment for each entry. I had similar 
ideas before but none that were easy to implement. A simple comment is not very 
hard:

1. add comment field to database
2. add comment flag to preferences to enable comments
3. add comment editing to edit dialog
4. add comment editing to create dialog
5. add comment display to measure list

I'll see that I implement this for the next round of enhancements
