Misc doc changes #91

Besides other changes, this commit ensures the generated HTML doc for HexDocs.pm will become the main source doc for this Elixir library which leverage on ExDoc features.

List of changes:

Use and set latest ex_doc
Update gitignore
Update formatter config
Fix typos
Use common source url
Add source reference
Refactor project config
Add changelog to html doc
Add missing changelog release dates
Fix markdowns
Badges and more badges!
Add license section
