Add keptn example to the examples directory to demonstrate chained-executor i.e. helmrelease + keptn executor.

[ ] Add prometheus-service & job-executor-service to keptn chart
[ ] Create SLO/SLI based on Istio sidecar delay metrics
[ ] Add istio fault-injection YAMLs to bookinfo chart
