Update VM & remove workarounds in setup #509

Description and Motivation

Random packages were causing issues in our update script when running apt-get upgrade. This PR fixes this in a number of ways:

Switch to 18.04-LTS which gets updated rather than point in time 16.04.0.
Remove the apt-get update from being done as part of vm extensions.
Switch to using cloud-init approach which is the standard for updating VMs of all types https://docs.microsoft.com/en-us/azure/virtual-machines/linux/using-cloud-init.

Github issue detailing use of cloud-init and pointer of how to do it in arm template https://github.com/MicrosoftDocs/azure-docs/issues/9282.
