Replace 'azure-chaos-fn' with 'platform-chaos' #29

Throughout this repository 'azure-chaos-fn' is still being used even though the official package is 'platform-chaos'. This needs to replaced to eliminate confusion.

Some example locations include README.md as well as the Wiki tutorial.
