migrate content from use_bluesky repository

I see advantages in merging the use_bluesky repository into this one:

• environments are defined there but used here - coordinate them
• the instrument package here is a better template than in use_bluesky -- reduce the duplication
• emphasize the use of environments and version control by this example
• this repo demonstrates the main UIs:
  • IPython console
  • Jupyter notebook
  • bluesky-queueserver
