Table of Contents missing #3

WP 4.4.1, PB 3.1.2, PBT 2.1.2, PmPDF 1.0.1 Export to PDF does not create a TOC, even when Table of Contents > Display table of contents is enabled. No errors generated.

Also noteworthy that Part > Hide from table of contents and part numbering - is being ignored and page numbers are in the footer of the Parts pages.

Additional research: Testing with WP 4.3.1 and PB 2.7.2, PBT 1.2.16 I have found that the mPDF did not export the Table of Contents. I was able to get a TOC from Pressbooks 2.7.1 and PBT 1.2.15. I also upgraded PBT to 2.1.2 and had a TOC was generated in PDF export (despite error message "PBT cannot find a Pressbooks install.").

I upgraded WP, PB, PBT, and mPDF to latest on test site. Result was that TOC was generated in PDF export. After testing, no plugins could be found to be incompatible on test site. Observation: The only major difference between test site (Working TOC) and issue site (TOC not working) is file and folder permissions. TOC works when:
