PDF export does not display Table of Contents

Description

I checked the PDF option to display Table of Contents, but it did not appear in the PDF export

Expected behaviour

Open Textbooks Theme Options > PDF options > checked box to "Display Table of Contents", and I expected the TOC to appear in the PDF export for print, as it did in the past

Actual behaviour

No TOC appeared in my PDF export for print. Download my test PDF from this fresh install of PBT/PB/WP https://pressbooks.trinfocafe.org/lifeinwesthartford

Steps to reproduce the problem

Check the box to Display table of contents

System Information

• Textbooks for Pressbooks: 4.0.2
• Pressbooks: 5.1.0
• WordPress: 4.9.4

Thank you for reporting this @JackDougherty — I'm not able to recreate this issue. I've tried on a couple of books and it successfully displays the TOC in the print PDF.

Is this still an issue for you? If so, can you confirm that there is no change when:

• you've toggled the Display TOC on/off, re-saving PDF options in between?
• you've switched to a new theme, and then re-activated the Open Textbook theme?
