Changes to DBT card

Can we make the following changes to DBT diary card permanent:

Currently we have the following 'Urges' questions in every diary card:

We want to add the following 'Urges' as a permanent feature in every diary card This should be in form of a matrix instead of individual questions per screen.
Once we have these set up, we can remove the earlier two questions because they are included in this.

Also, the following questions about 'Emotions' should be permanent in form of a matrix. We still want to give users the option to create new ones, but these should be included in every new DBT diary card.

I just tested it in staging. For the 'urges', we have 'die by suicide and 'quit therapy' hardcoded. We also need to add the remaining 4 ("die", "suicide", "use/drink", self-harm").

Also, Is it possible to make adding 'emotions' while making a new diary card optional? Right now, you have to add at least one emotion (in addition to the one hardcoded) for the DBT diary card to work.

More changes requested:

1. Change the scale for emotions and targets to read: '0= Not at all- 5= Extremely Strong' instead of '0= Not at all- 5= Extremely'.
2. In Emotions, change 'sad' to 'sadness'
3. In Urges, remove 'Use/Drink' and 'Suicide'

Can we add the following text below each section like we have for emotions (example in the screenshot):

Effective Target Behavior:

Ineffective Target Behavior: Default Ineffective Behaviors: Die, Self-Harm, Quit Therapy, Die by Suicide. Do not add these below. Emotions: Default Emotions: Sadness, Shame, Anger, Fear/Worry, Joy. Do not add these below.

@Devayani0102 Can we just use the strings Both good and bad emotions and default emotions string in single line. ? We are using rjsf form, and we don't have option to use 2 descriptions.

After the latest release, the changes made before oct 6th do not show up in production. The fix seems to be working in staging.

@Devayani0102 @ertjlane @carlan1 @michaelmenon @ZCOEngineer LAMP-activities are not released to production. Last update was done on Aug 4. If you can release that to production, you can see the changes.
