Legacy 2FA web UI Visible in non-BIDMC Servers #752

Describe the bug Per the description of issue #736 , the legacy 2FA ui should only appear if users enter into server.lamp.digital. However, it has appeared on multiple other server addresses (ex: pep.api.mayo.edu/mlpapiservice/v1, mindlamp-api.pronet.med.yale.edu). This could cause tremendous confusion among non digital psych lamp users. This needs to take priority over other efforts until it is resolved.

To Reproduce Go to any non-server.lamp.digital mindLAMP server and log in with administrative credentials.

@ertjlane , sure we will check on it and update by Monday. Should be a small tweak.

@ertjlane We have updated the fix in dashboard-staging. Could you please check from your side as we don't have any credentials for non-digital psych lamp users?

This appears to be fixed in staging!

I'm making a release for the dashboard to push this to production. Make sure to attach the 'latest' tag to the new release. Thanks!

Appears fixed. Will close issue
