I'm fairly sure you're using an old daemon version. Current version should be 1.1.2 Using an old daemon will mean you're on the old chain from the recent hard fork.

Transaction was probably in the mempool on that node only and not actually sent. Did upgrading to new version resolve this issue @jRazer ?
