Prescription Upload Page has 2 fields #5

Whenever you try to upload a prescription, there are 2 fields to upload them.

it wasn't taking any upload from the designed one so tried with that, also will fix it with aryan because the thing doesn't work for me but it does for him 😩 @im-NL
