Wrong formating of "virtual server" settings page on dir320

What steps will reproduce the problem?
1.On WebAdmin go to natsettings->virtual server 
2. Page formatting and some controls is missing
3. Not possible to setup port forwarding(vbrtual server) using webadmin

What is the expected output? What do you see instead?
Same page looks fine on Asus router with same version of firmware 

What version of the product are you using?
D-Link Dir-320 firmware 3.0.4 4721+

Please provide any additional information below.
This issue appeared when new web interface(with ENTWARE logo) was released in 
dir320 version of firmware. I tested the issue on different browsers and with 
default settings.
