Logout Prompt Missing CSS Style Template #5

When a prompt is enabled, the dialog display is not using the common login CSS and renders at the top of the page.

The Prompt should be centered, and a back button should be provided to allow the user to return to the prior interface (e.g. history.go(-1))

Note also that the "back" button could be a "No" button to the right of the "Yes" button.
