resolve conflicts in picture names #190

in Quito they were making sure the picture names would never conflict: pictures were renamed to something containing the accession code before storing them in the database. it is possible though, in particular if an institution takes pictures with different cameras of the same type, that pictures are added which have a name that has already been used.
