I frequently cannot put items in exchange machines (I don’t think this happens with vending machines). I try to put an item in, and it just doesn’t go in the slot. I’m able to fix this by putting a different item in, putting the item I wanted to put in the machine into a different slot, and then (optionally) removing the different item, but this happens again once I put a new item in, so doing this workaround constantly when filling up an exchanging machine gets really tedious.
This happens in both single-player and LAN multiplayer.
