With the lates v1.31 beta I still have a problem, namely when I add references during the track changes mode and then accept the changes. To reproduce, do the following
Create a BibTeX entry in your BibTeX file
@InProceedings{Al-Jarf2009,
Title = {Enhancing Freshman Student's Writing Skills with a Mind-Mapping Software},
Author = {Al-Jarf, Reima},
Booktitle = {Proceedings of eLearning and Software for Education (eLSE)},

In Word, create a new document and activate "Track Changes"
Add the reference...
Press the "Accept" button twice (the first click will select the change, the second click will accept it)
